DMCCU stands for Disease Management Centralized Care Unit (DMCCU). The Healthy Blue DMCCU program gives you a choice and a voice in how to care for your health. DMCCU case managers are licensed nurses. With their help, you will learn to better manage your condition and improve your quality of life.

Bipolar disorder

Bipolar disorder happens in a person’s brain. It can cause unusual changes in mood, energy and level of activity, and make it hard to carry out routine day-to-day tasks.

A person who lives with bipolar disorder can have extreme mood swings. They can have long periods of feeling “high” or overly happy. This is called a manic episode.

They can also have periods of feeling very low, down, worried or hopeless. This is called a depressive episode. These feelings can make it hard to concentrate and to stay in touch with family and friends.

What can you do if you think you might be living with bipolar disorder?

Talk to your primary care provider (PCP). They can help you get a proper diagnosis. With treatment and support, people with bipolar disorder can live healthy, happy and productive lives.

Healthy Blue can help you with your condition. Services include:

  • One-on-one help with depression, schizophrenia, bipolar disorder or substance use disorder
  • Treatment from a licensed mental health caregiver
  • Help finding a provider
  • Information about resources in your area
  • Phone calls to you and your doctors to track your progress

Healthy Blue covers health care and support services based on your heart condition like coronary artery disease (CAD), hypertension and congestive heart failure (CHF).

Services include:

  • Heart health education materials and one-on-one support with a nurse
  • Medical equipment you need, such as a scale, to help you manage your health
  • A home health nurse when needed for diet help and when and where to go for care
  • Information about heart education programs in your area
  • Phone calls to you and your doctors to track your progress

Healthy Blue has a case management program for HIV/AIDS. This program will help you better understand and manage your HIV/AIDS. We can help you to set health goals and create a care plan that fits your lifestyle. You do not have to join the program. You are enrolled as a member of Healthy Blue.

Healthy Blue covers health care and support services based on your lung condition. Lung conditions include asthma and chronic obstructive pulmonary disease (COPD).
